www.tagmobile.net or an authorized TAG Mobile agent. The Lifeline form verifies that you have not received a Link-up credit at your residence and that you participate in at least one of the following government subsidized programs:
- Food Stamps
- National School Lunch’s Free Lunch Program (NSL)
- Section 8 Federal Public Housing Assistance (FPHA)
- Medicaid
- Supplemental Security Income (SSI)
- Temporary Assistance for Needy Families (TANF)
- Low Income Home Energy Assistance Plan (LIHEAP)
- Income at or below 135% of federal poverty level (proof required)
